Fleming Frogs Soccer: WAIVER OF LIABILITY

THIS AFFECTS YOUR LEGAL RIGHTS AND YOUR CHILD’S LEGAL RIGHTS

The Fleming Frogs soccer program is for children 3 - 5 and will consist of practices and games. These events and activities are strictly voluntary and the decision to participate in them is solely at the discretion of the parent of each participant.
As a legal parent or guardian of the participant, I understand that participation on the part of my child is strictly voluntary on my child’s part, and that he or she may be injured by participating. By taking any part in these activities, I, on behalf of my child and myself, hereby release and forever discharge the organizers of the program, the coaches, and any other volunteer who is or will be involved in the running and organizing of the program, from any and all claims and liability, whether arising out of negligence or otherwise, for injury, loss, and damage, including personal injury or death and damage to property or loss of income, that may arise from my or my child’s involvement in these events or activities, whenever and however they may occur.

Fleming Frogs Soccer: WAIVER OF MEDIA USAGE

The Fleming Frogs League will take a team picture at the start of the season, to be printed and given to you at the end of the season. Throughout the summer, your child may be photographed and photos may be placed on our website.

Refunds will not be issued.
